Client Consent & Health Assessment Form

1. Nature of Treatment

Manual therapy services provided by Circadex Performance may include sports massage, deep tissue massage, soft tissue release, myofascial release, cupping therapy (vacuum cup application), and manual joint mobilisation techniques. Each session will be discussed and agreed before commencement.

2. Potential Risks & Side Effects

Manual therapy is generally very safe. However, the following effects may occur:

• Common (minor): Temporary soreness or tenderness in the treated area (typically 24–72 hours), fatigue, mild headache, temporary increase in symptoms before improvement.
• Cupping therapy: Circular skin discolouration (bruise-like marks) lasting 3–10 days, temporary skin redness, mild skin sensitivity, and in rare cases small blisters.
• Joint mobilisation: Short-term stiffness or achiness, audible "clicking" sounds (normal), and in rare cases a short-term increase in discomfort.
• Rare but serious: Allergic reaction to massage oils; nerve irritation; vasovagal response (dizziness/fainting); exacerbation of an undiagnosed condition. Inform your therapist immediately if you feel unwell during treatment.

3. Contraindications & Client Responsibility

Certain medical conditions may mean that all or part of a session cannot be carried out safely. Circadex Performance reserves the right to decline or modify treatment where contraindications are identified. You are responsible for providing full and accurate health information. Failure to disclose a relevant condition may affect the safety of your treatment and any subsequent liability claim.

4. Limitation of Liability

Manual therapy is not a substitute for medical diagnosis or treatment. Circadex Performance does not diagnose medical conditions. Liability for any adverse outcome is limited to circumstances where negligence or malpractice on the part of the therapist is proven. No liability is accepted for adverse outcomes resulting from inaccurate or incomplete health information provided by the client.

5. Right to Withdraw Consent

You have the right to withdraw consent at any time, including during a session, without obligation or penalty. Simply inform the therapist and treatment will stop immediately.

6. Confidentiality & Data Protection — UK GDPR / Data Protection Act 2018

Your personal and health data is collected solely for safe and appropriate treatment and record-keeping. Circadex Performance confirms your information will:

• Be stored securely and not shared with third parties without your written consent, except where required by law.
• Be retained for a minimum of 7 years (or until age 25 if the client is a minor) in accordance with professional standards.
• Be accessible to you on request at any time.
• Not be used for marketing purposes without your separate, explicit consent.

You have the right to access, rectify, or request deletion of your data at any time by contacting Circadex Performance directly.

7. Professional Standards

Your therapist operates within their defined scope of practice and in accordance with relevant professional codes of conduct. Treatment will be conducted with full respect for your dignity, privacy, and modesty, with appropriate draping used throughout all sessions.
